Abstract

Recent developments in functional and digital dentistry have significantly transformed the diagnostic and therapeutic landscape. Cutting-edge technologies, such as intraoral and facial scanning, dynamic cone beam computed tomography (CBCT), and tracking of free mandibular movements, now enable dental professionals to gather highly detailed and accurate data about their patients. These innovations have elevated the precision of clinical assessments and treatment planning. By integrating a neuromuscular approach with 4D digital technology, practitioners can achieve superior levels of predictability, precision, and personalized care, tailoring treatments to align with the physiological parameters and unique preferences of each patient.

This approach is exemplified in the case of a 36-year-old female patient who sought care following orthognathic surgery. Her complaints included difficulties in mastication, dissatisfaction with dental aesthetics, recurrent debonding of an upper retainer, and the presence of carious lesions in posterior teeth. A comprehensive neuromuscular evaluation was conducted, incorporating surface electromyography (sEMG) of key muscles, digital recordings of free mandibular movements, and integration of intraoral and facial scans with full-head CBCT imaging. These data were processed in CAD software following a structured 4D evaluation protocol.

The treatment plan focused on mandibular deprogramming, dynamic functional analysis, occlusal stabilization via an orthotic appliance, and final prosthetic rehabilitation using minimally invasive crowns and veneers. This systematic approach aimed to optimize both functional and aesthetic outcomes. The patient provided consent for the proposed treatment plan and the use of clinical documentation for academic purposes.

Digital dentistry combined with neuromuscular principles offers a revolutionary approach for precise, personalized care, enhancing diagnostic accuracy and treatment outcomes.
